Job Description

Melbourne City Mission is seeking a dedicated Case Manager to join our youth services team in Melbourne. You will work with Youth Refuge Workers and a multidisciplinary team to deliver high quality case management and coordinate housing and support services for young people in Circuit Breaker Refuge.

You will support pathways out of homelessness, develop care plans, perform risk assessments, and oversee care coordination with external providers.
