The role involves providing patient assessments, wound care, immunizations, care planning, and other clinical nursing services under appropriate protocols and guidelines.

Day-to-day tasks include assisting with chronic disease management, conducting health screenings, supporting minor procedures, and maintaining accurate clinical documentation.

The Practice Nurse will collaborate closely with general practitioners and other health professionals, contribute to patient education, and promote preventive health initiatives. The role also includes participation in quality improvement activities, adherence to clinical safety standards, and effective communication with patients, families, and colleagues.

Key Responsibilities

- Support GPs and coordinate patient care and follow-up
- Conduct patient assessments, care planning and health education
- Assist with chronic disease management, vaccinations and wound care




- Maintain accurate clinical records and infection control standards
- Contribute to quality improvement and practice initiatives
- Work collaboratively with the clinical and administrative teams

Skill & Experience

- Current AHPRA/NMBA Registered Nurse registration
- Experience in general practice, primary care or a similar setting
- Robust clinical assessment, documentation and communication skills
- Knowledge of immunisation, infection control and chronic disease management
- Commitment to person-centred care, cultural safety, privacy and confidentiality
- Ability to prioritise effectively and work calmly in a busy clinical environment
- Current CPR/BLS certification
- Vaccination competency and relevant clearances are desirable
