About the Role

n

The Disability / Aged Carer is responsible for providing high-quality, person-centred care and support to people living with disability and/or older people, enabling them to live as independently and safely as possible.

n

The role involves supporting participants with their daily living needs, personal care, community participation, social engagement, household activities, and other supports identified in their individual support plans.

n

Key Responsibilities

n

n
- Provide respectful, individualised and person-centred support based on each participant's needs, preferences, goals and support plan.
n
- Promote participant choice, control, dignity, independence and participation in decision‐making.
n
- Provide personal care and assistance with activities of daily living, where required and within the worker's training and scope of duties.
n
- Assist participants with showering, dressing, grooming, toileting and continence support as required.
n
- Support participants to access community, recreational, educational, vocational and social activities.
n
- Accompany participants to appointments, shopping, social activities and community events as required.
n




- Promote inclusion and participation while respecting each participant's choices and preferences.
n
- Follow individual support plans, health plans, risk assessments and behaviour support plans as applicable.
n
- Monitor and report changes in a participant's health, wellbeing, behaviour or support needs.
n
- Support participants in accordance with their individual behaviour support plans where applicable.
n
- Use respectful, calm and person-centred approaches when responding to behaviours of concern.
n
- Maintain participant dignity and safety when responding to challenging situations.
n
- Communicate effectively with Team Leaders, Support Coordinators, Service Managers, families, carers and other relevant stakeholders.
n

Requirements

n

n
- Previous experience working with people with disability, older people, or other individuals requiring care and support.
n
- Demonstrated understanding of person-centred care and support.
n
- Ability to support participants with personal care and activities of daily living.
n
- Solid communication and interpersonal skills.
n
- Ability to work respectfully with people from diverse cultural and personal backgrounds.
n
- Ability to follow individual support plans, procedures and instructions.
n




- Ability to maintain accurate documentation and progress notes.
n
- Ability to recognise and report risks, incidents and changes in participant wellbeing.
n
- Ability to work independently and as part of a multidisciplinary team.
n
- Good organisational and time-management skills.
n
- Current and valid relevant State/Territory Driver's Licence, where required for the position.
n
- Valid First Aid and CPR Certification.
n
- Current NDIS Worker Screening Check, where applicable.
n
- Working with Children Check, where required.
n
- Ability to meet the inherent requirements of the position, including any relevant physical and manual handling requirements.
n
- Certificate III or IV in Individual Support, Disability, Ageing Support, Community Services or a related qualification.
n
- Previous experience in disability, aged care, community care, supported independent living or accommodation services.
n
- Experience supporting people with complex physical, intellectual, psychosocial or behavioural support needs.
n
- Experience supporting participants with complex health needs.
n
- Experience working with people with dementia or age-related conditions.
n
- Knowledge of NDIS Practice Standards and person-centred approaches.
n
- Knowledge of aged care standards and principles.
n
- Experience implementing positive behaviour support strategies.
n
- Current medication administration training, where relevant to the role.
n
- Manual handling training.
n
- Experience working in SIL, group home, respite, day program or in‐home support environments.
