Aboutthe Role

The Enrich Program supports young people aged 12–25 in Sunbury and the Macedon Ranges with complex mental health and psychosocial needs through assessment, care coordination, therapy and documentation.

The Enhanced Care Coordinator provides assessment, intervention and referrals for young people with moderate to severe mental health concerns who fall between tertiary and mainstream supports.

We are seeking an Enhanced Care Coordinator to join our multidisciplinary counselling team in the Enrich Program. The successful applicant will be responsible for managing a diverse case load of young people with complex mental health presentations.

The role will see the successful applicant collaborating with other health professionals across the organisation.



You will be responsible for providing services to clients within the Macedon Ranges and Sunbury areas. The role is responsible for providing assessment, therapeutic interventions and case coordination for clients face-to-face in a variety of settings, collaborating with other health professionals and managing a diverse case load.

About You

- Minimum of three years’ experience in a mental health setting
- Bachelor of Social Work, Bachelor of Occupational Therapy, Bachelor of Nursing, or Master of Psychology
- Registration or eligibility for registration with a recognised professional body (e.g. AHPRA, AASW, APS)
- Must hold current National Criminal History Check, NDIS Worker Screening check and valid Working with Children Check
- Must hold current Victorian Driver’s Licence.
