Seasons is seeking a compassionate and experienced Clinical Care Partner to join our team at our beautiful Sinnamon Park community on a full-time basis.

This is an exciting opportunity for a care professional with experience in aged care, home care, case management or clinical coordination who thrives on building strong relationships and delivering exceptional resident outcomes. Working closely with residents, families, clinical teams and external providers, you will play a pivotal role in coordinating person‑centred care, supporting residents to achieve their goals, and ensuring services remain aligned with their changing needs, preferences and funding arrangements.

You will:

- Manage a portfolio of residents receiving Support at Home, DVA and private services, ensuring care plans remain person‑centred, responsive and outcome‑focused.
- Conduct assessments, develop and review care plans, and coordinate six‑month, annual and change‑in‑condition reviews.
- Support residents and families to navigate care options, funding arrangements and service pathways, acting as a trusted point of contact.
- Monitor resident wellbeing, identify and escalate clinical risks, and collaborate with Registered Nurses, GPs, allied health professionals and other stakeholders to achieve positive outcomes.
- Coordinate specialised programs and pathways, including Restorative Care, End‑of‑Life Care, Assistive Technology and Home Modifications, and DVA‑funded services.
- Maintain accurate documentation, monitor budgets and service utilisation, and ensure compliance with aged care legislation, quality standards and organisational requirements.

- Qualifications in aged care, nursing, health, community services or a related discipline.
- Experience in care coordination, case management, clinical support or home care services.
- Robust knowledge of person‑centred care, wellness and reablement principles.
- Excellent communication and relationship‑building skills.
- The ability to manage competing priorities in a fast‑paced environment.
- Confidence using electronic care management systems and Microsoft Office applications.
- Unrestricted Australian working rights and a current National Police Check.
