Job Description

East Grampians Health Service invites a part time Carer Support Program Coordinator to join Community Services - EGHS @ Home in Ararat. You will identify carers' needs, respond with suitable services, and facilitate informal group meetings, advocating for carers and expanding the program through referrals.
