Key responsibilities

Community access and social support

Personal care and daily living assistance

Participant mentoring and confidence building programs

Supporting independence and life skills

Progress notes and clear documentation





Collaboration with management, families and stakeholders

About you

Confident communicators who build rapport easily

Reliable, proactive and solution-focused

Motivated to learn and grow within the sector

Full Australian driver's licence and reliable vehicle

Blue Card & NDIS Worker Screening Check (Yellow Card)

National Police Check

First Aid & CPR

Excellent written and verbal English skills (clear documentation and incident reporting required)
