Join the dedicated team at Trinity Adventure Projects as a Support Coordinator, providing essential support and guidance to young people and their families in relation to their NDIS services. In this full-time role, you will play a pivotal part in empowering vulnerable individuals and families, helping them navigate challenging circumstances and achieve positive, sustainable outcomes.

Key responsibilities

- Manage a caseload of young people and their families, conducting comprehensive disability needs assessments and developing tailored support plans
- Coordinate a range of specialised disability services, including referrals to partner organisations, and practical assistance
- Collaborate closely with other professionals, such as social workers and youth workers, to ensure holistic, wrap-around care
- Advocate for clients' rights and ensure they have access to appropriate resources and opportunities
- Maintain detailed case notes and records, contributing to the continuous improvement of service delivery
- Participate in team meetings and career development activities to enhance your skills and knowledge

About you

- Tertiary qualification in human services, criminology, social work,



psychology, or a related field
- Minimum 2 years' experience working in a similar case management, youth/family support role or support coordination role
- Demonstrated understanding of the challenges facing young people and families in vulnerable situations
- Knowledge of NDIS processes, Support Coordination principles, and disability service frameworks
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse clients
- Proven ability to work collaboratively within a multidisciplinary team
- Strong organisational skills and attention to detail
- Passion for making a positive difference in people's lives
- Range of wellbeing initiatives to support overall health and happiness
