As a key member of the ATSICHS Brisbane team, contributing to excellent patient care the Care Coordinator Nurse supports patients with Chronic Illness.

The Care

Coordinator is responsible for the provision of effective service coordination working closely with clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people.

Duties

- Provide care coordination to assist in the provision of chronic disease management.
- Establish and maintain relationships with key external stakeholders conducive to holistic health and social support needs.
- Foster a culture of continuous improvement and innovation within the Primary Health Support Services, integrating culturally safe practices.




- Ensure effective implementation and maintenance of ATSICHS Policy and procedures in line with AGPAL Accreditation Standards.

- Starting salary of $78,447 + 12% super.
- Permanent full time role.
- Salary package up to $15,900 net and add Entertainment Advantages up to $2649 every Fringe Benefits Tax (FBT) year.

Required

Desired Skills and Experience

- Minimum Bachelor in Nursing.
- Full un-restricted Registered Nurse AHPRA registration.
- Previous demonstrable experience in relevant nursing field.
- Demonstrated ability to meet targets and performance outcomes.
- Ability to support a continuous improvement model for achieving outcomes.
- Decisive listening, influencing and persuasive skills.
- National Police Certificate.
- Driving Licence.
- AHPRA Registration.
- Evidence of Work Rights.

Preferred

- Minimum two (2) years’ experience in a primary health setting.

- Previous experience of working with Aboriginal and Torres Strait Islander clients.
- Previous experience in a multidisciplinary health team.
- Previous experience in chronic disease management.
