About the role

We are seeking dedicated Care Service Employees to join our team. In this hands-on role, you will deliver safe, high-quality, person-centred care that supports residents' independence, dignity and wellbeing, in line with Evergreen's mission and HEART values.

Key responsibilities

- Assist with daily living activities, including personal care, meals, hydration, mobility and transfers.
- Monitor residents' wellbeing, report changes and complete required observations in line with care plans.
- Accurately document care and resident responses in clinical records.
- Assist with basic clinical tasks and medication support under the direction of a Registered Nurse, within scope.
- Use care equipment safely and maintain a clean, safe and supportive environment.
- Support residents to participate in societal activities and outings.

About you

- A Certificate III in Individual Support or Certificate IV in Aged Care.
- Strong interpersonal and communication skills, with the ability to build positive relationships with residents, families and team members.
- Minimum 6 months experience supporting personal care and daily living activities in an aged care or health setting.
- The ability to work within scope of practice, follow care plans, and take direction from Registered Nurses.
- A commitment to resident privacy, confidentiality and safe work practices.
- The ability to work both independently and as part of a flexible, multi-skilled team.
- A current National Criminal Record Check (or willingness to obtain one).
- Willingness to participate in a pre-employment medical assessment.
