About Us The CREW Rockingham is a local not-for-profit organisation that has been serving the Rockingham community since 2015. What began as a small community response to people experiencing hardship has grown into a well-established frontline service supporting people experiencing homelessness, housing instability, isolation and disadvantage across our community.
We believe in meeting people where they are, building genuine relationships and providing practical support that helps people take their next step. Our work includes food relief, community connection and outreach, showers and laundry, emergency support and our Safe Night Space, providing overnight crisis accommodation for up to 30 guests each night.
The CREW is more than a service provider. We are a community of people who care deeply about seeing individuals treated with dignity, finding connection and moving towards greater stability and independence.

About the Role
We are seeking two Case Managers to join The CREW Rockingham team. We are currently recruiting for full-time positions, however, we are also open to discussing part-time employment options for the right applicants.
Based in Rockingham, WA 6168, these roles are central to our mission of supporting individuals and families experiencing homelessness and housing insecurity. As a Case Manager, you will provide holistic, person-centred support to people experiencing complex circumstances, coordinating case management that responds to immediate housing needs while building pathways towards sustained independence and stability.
These roles require someone who is compassionate and understanding, while also being practical and solution-focused. You will work collaboratively with clients, community services, housing providers and other stakeholders to navigate barriers, advocate for clients and help achieve meaningful, sustainable outcomes.
Key Responsibilities
- Undertaking comprehensive assessments of clients' housing, health, financial, and psychosocial needs to develop tailored support plans
- Providing ongoing case management support, including regular client contact, monitoring progress against agreed goals, and adjusting support plans as needs evolve
- Connecting clients with appropriate services, accommodation options, and community resources, including housing providers, mental health services, financial counselling, and employment support
- Advocating on behalf of clients with government agencies, landlords, service providers, and other stakeholders to secure necessary support and accommodation
- Supporting clients through housing applications, tenancy negotiations,
and maintaining tenancies through proactive problem-solving and intervention
- Maintaining detailed and accurate case records and documentation in compliance with organisational policies and legal requirements
- Collaborating with internal team members, partner organisations, and external stakeholders to coordinate integrated support services
- Identifying clients' strengths and building on these to promote self-efficacy, resilience, and independence
- Assisting clients in accessing crisis support services when needed and providing emotional support during periods of distress or setback
- Contributing to the development and continuous improvement of service delivery practices
What We're Looking For
- Minimum two to three years' experience in case management, community services, housing support, or a related human services field
- Demonstrated understanding of homelessness, housing insecurity, and the complex factors contributing to these issues
- Strong interpersonal and communication skills with the ability to build rapport and trust with vulnerable clients from diverse backgrounds
- Excellent organisational and time management abilities, with capacity to manage multiple cases and competing priorities effectively
- Proficiency in recognising and responding appropriately to mental health, substance use, and trauma-related issues
- Knowledge of community services, support providers, and housing options available in the Perth metropolitan area and Western Australia
- Ability to work independently whilst maintaining strong collaborative relationships with colleagues and partner agencies
- Sound problem-solving skills and the ability to think creatively about supporting clients facing complex, multifaceted challenges
- Demonstrated commitment to social justice principles, cultural sensitivity, and non-discriminatory practice
- Reliable, conscientious approach with strong ethical standards and professional boundaries
- Current driver's licence and access to reliable transport (for home and community visits)
- Willingness to undertake training and professional development as required
- Relevant qualifications such as a diploma or bachelor's degree in social work, community services, health, or related field are highly desirable
