Hours: Monday – Friday, 9:00 AM – 5:00 PM

We are currently looking for two motivated and experienced professionals to join our team:

CASE MANAGER – SUPPORT AT HOME & NDIS

Requirements

Minimum 1 year of experience in case management, Support at Home, NDIS, aged care, disability services or a similar field (preferred)

Fluent in Arabic and English

Current Driver’s Licences

Strong communication, organisation and relationship-building skills

Ability to work independently and as part of a team

Key Responsibilities

Develop, implement and manage Support at Home packages and NDIS plans

Build and maintain strong working relationships with clients, families, allied health professionals and other stakeholders

Partner with consumers to develop and implement consumer-driven care plans

Provide ongoing support and monitor client needs

Ensure clients understand their budgets and that packages are managed effectively within budget

Maintain accurate, timely and up-to-date documentation in accordance with company policies and procedures





REGISTERED NURSE

Requirements

Current AHPRA registration

Experience in community, home care, aged care or a similar healthcare workplace

Fluent in English

Compassionate, respectful and committed to person-centred care

Strong clinical assessment, decision-making and communication skills

Ability to work independently while contributing effectively to a team

Organised, reliable and able to manage competing priorities

Professional and committed to maintaining client confidentiality and dignity

Strong ability to build positive relationships with clients, families and healthcare professionals

Comfortable using clinical documentation systems and maintaining accurate records

Committed to ongoing professional development and maintaining current clinical knowledge

Able to work in accordance with workplace health and safety requirements

Strong team collaboration skills
