About the Role

n

As an APS6 Support Needs Assessor, you will conduct high-quality, evidence-based assessments to understand participants' individual support needs and circumstances. Drawing on your clinical expertise, you will gather and interpret information, engage with participants and their support networks, and prepare transparent, consistent and defensible assessments.

n

You will take a person-centred, strengths-based and trauma-informed approach, ensuring participants feel respected, heard and supported throughout the assessment process. Assessments may be delivered virtually or face-to-face across a range of settings.

Key Responsibilities

n

n
- Conduct support needs assessments with participants and representatives
n




- Analyse participant information and external evidence to inform assessments
n
- Prepare clear, accurate and evidence-based written assessment reports
n
- Apply relevant policies and procedures using sound professional judgement
n
- Communicate complex or sensitive information respectfully and clearly
n
- Collaborate across teams, elevate risk and provide up the service improvements
n

n

About You

n

To be successful in this role, you will have:

n

n
- A recognised tertiary qualification in Allied Health or related disciplines, such as:n n
- Occupational Therapy
n
- Physiotherapy
n
- Social Work
n
- Nursing
n
- Counselling (or similar)
n

n
- Demonstrated experience in conductingsupport needs assessments
n
- Solid commitment toperson-centered, strengths-based practice
n
- Excellent communication and stakeholder engagement skills
n
- Well-developed analytical and decision-making capabilities
n
- Sound knowledge ofdisability legislation, policy, and operational frameworks
n
- Proven ability to work sensitively with diverse communities usinginclusive and trauma-informed practice
n

Australian Citizenship is a mandatory requirement for this position.
