Your chance The Clinical Nurse Chronic Disease is a clinical nurse who uses high-level clinical and problem-solving skills. They provide evidence based coordinated care to individuals and groups of chronic disease clients including those with type 2 diabetes, cardiovascular disease, chronic respiratory disease and renal disease.

You will use leadership in clinical decision-making to provide and/ or coordinate care to the clients and their carers/ families living within St George and surrounds.

St George Is a Designated Remote Area Nursing Incentive Package (RANIP) Site In Accordance With The Nurses And Midwives (Queensland Health) Award – State 2015.
