Uniting Victoria and Tasmania Horsham is seeking an AOD Home-Based Withdrawal (HBW) Nurse to join our team and support people on their journey to recovery in a home-based, non-residential setting.
You will conduct clinical assessments, monitor withdrawal, assist with medication management, and coordinate care with GPs, hospital teams and mental health clinicians to deliver holistic, client-centred support outside hospital settings.
