Queensland State Manager - Care at Home

n

The Opportunity

n

Reporting to the National Director, Operations, you will lead a growing portfolio of in-home and virtual healthcare services, including Hospital in the Home, rehabilitation, palliative care, chronic care and other hospital-substitution models.

n

Working closely with clinical, operational and executive leaders, you will oversee service delivery, performance and growth, ensuring care remains safe, high quality, financially sustainable and centred on patient needs.

n

You will also play a key role in connecting Care at Home services with the broader healthcare system, building strong partnerships with hospitals, clinicians, private health insurers, government and other funding partners.

What You'll Do

n

n
- Lead the delivery, growth and optimisation of Care at Home services across Queensland
n
- Provide operational leadership across multidisciplinary clinical and operational teams
n
- Develop and implement strategies that support sustainable growth, quality and performance
n
- Partner across Clinical, Product and Growth teams to design and scale home-based and virtual care models
n
- Ensure services integrate effectively with hospital pathways, operating models and funding arrangements
n
- Oversee workforce planning, service delivery, resource allocation and operational performance
n
- Monitor clinical, financial, quality and patient outcomes to drive continuous improvement
n
- Maintain strong clinical governance, risk, compliance and accreditation standards
n
- Build effective partnerships with hospitals, clinicians, insurers, government and other key stakeholders
n
- Lead and develop high-performing teams while fostering a collaborative, accountable and innovative culture
n

About You

n





You are an experienced healthcare leader who combines strong operational capability with a genuine interest in healthcare transformation and improving the way care is delivered.

n

You understand the complexities of delivering clinical services within, or closely alongside, hospital environments and are comfortable working across multiple funding arrangements, stakeholder groups and models of care.

n

You are equally comfortable leading day-to-day operations and supporting service growth. You can establish new ways of working, solve operational challenges and translate strategy into practical outcomes as services continue to develop.

n

You will bring commercial awareness, sound judgement and a strong commitment to high-quality, person-centred care.

n

Key Experience & Capability

n

n
- Senior leadership experience across healthcare operations and multidisciplinary teams
n
- Strong operational, financial and performance management capability
n
- Experience across acute care, hospital substitution, community health, rehabilitation, virtual care or home-based healthcare
n
- Demonstrated success establishing, growing or improving healthcare services
n
- Strong understanding of healthcare funding, patient pathways, clinical governance, quality, risk and accreditation
n
- Proven ability to build high-performing teams and effective partnerships with hospitals, clinicians, funders and government
n
- Experience delivering service improvement, innovation and healthcare transformation
n
- Excellent stakeholder engagement, communication and influencing skills
n
- Ability to translate strategy into measurable operational outcomes
n

Desirable

n

n
- Experience in Hospital in the Home, rehabilitation, oncology, palliative care, cardiac services or related clinical services
n
- Exposure to both public and private healthcare environments
n
- Knowledge of private health insurance and state-based healthcare funding models
n




- Experience with virtual care, remote monitoring or other digitally enabled models of care
n
- Experience working across multidisciplinary allied health services
n
- Postgraduate qualifications in Health Management, Business Administration or a related discipline
n

Qualifications

- Tertiary qualifications in Nursing, Allied Health or another relevant clinical discipline are required
n
