Health Care Workers - Home/ Community Support

We are inviting applications from suitably qualified candidates to join our My Community Home Care (MCHC) team in the role of Health Care Worker.

n

As a Community Health Care Worker, you will provide person-centred care, assisting our consumers to remain living independently in their own homes by providing assistance with domestic and personal care, social support, respite, and transport.

n

Our service areas include Bethanga, Bellbridge, Dartmouth, Eskdale, Mitta Mitta and Tallangatta. Within close proximity to Albury/Wodonga, you can enjoy some of north-east Victoria's beautiful scenery whiletravelling between clients in a company supplied vehicle.

n

This is aMonday to Friday (no weekend work) roleand requires a current First Aid certificate or a willingness to obtain.

n

Key Responsibilities Include:

n

n
- Provide safe, person-centred care that promotes consumer independence and dignity.
n
- Assist with personal care, nutrition, hydration, continence care, and medication support as authorised.
n
- Maintain accurate documentation and communicate changes in consumer condition.
n
- Work in accordance with relevant legislation, regulatory requirements, quality standards, and organisational policies in aged care and/or community settings.
n

- Remuneration in line with the Health & Allied Services, Managers and Administrative Workers Enterprise Agreement ($30-$33/hr based on qualifications and experience)

About you

n

You are passionate about what you do and want to support our consumers to live life to their potential,



you also hold the following:

n

- Certificate III in Individual Support/ Aged Care or equivalent is mandatory.
n
- Current Drivers License
n
- A clear and strong commitment to the providing quality care to our Consumers/Residents
n
- Strong communication and consumer service skills
n
- Well-developed interpersonal skills
n
- Ability to prioritise tasks and problem solve
n
- Able to comprehend and follow written instructions and procedures
n
- Motivated and able to work effectively in a team environment and independently
n

All appointments are made subject to the required security screening including National Criminal History, NDIS Worker Screening checks andan immunisation status appropriate to the role, including the annual Influenza vaccination.

n

This is an onsite position that requires travel throughout surrounding communities.
