About the Role

Connecting Communities is seeking compassionate, reliable and person-centred Disability Support Workers to support NDIS participants to live an ordinary, meaningful and inclusive life.

As a Disability Support Worker, you will provide individualised support that is guided by each participant’s NDIS goals, preferences, choices, needs and support plan.

Your role will involve supporting participants to develop and maintain independence, exercise choice and control, participate in their community and achieve their personal goals.

Key Responsibilities

Depending on the participant’s individual support needs, responsibilities may include:

· Supporting participants to work towards their NDIS goals and individual outcomes.

· Providing person-centred support that respects each participant’s rights, dignity, privacy, culture, values, preferences and choices.

· Supporting participants with daily living activities and developing greater independence.

· Providing personal care and assistance with hygiene in a respectful, dignified and professional manner.

· Supporting community access, social participation and meaningful activities.

· Providing in-home support, including household tasks where these form part of the participant’s individualised support arrangements.

· Encouraging participants to make informed choices and exercise choice and control over their supports.

· Supporting participants to maintain relationships, community connections and social inclusion.

· Following participant support plans, risk assessments, medication procedures and organisational policies and procedures.

· Maintaining appropriate professional boundaries and protecting participant privacy and confidentiality.





· Recognising and responding appropriately to safeguarding concerns, incidents, hazards and changes in a participant’s wellbeing.

· Completing accurate and timely progress notes, shift documentation, incident reports and handovers.

· Working collaboratively with participants, families, support coordinators, health professionals and other members of the participant’s support team, where appropriate.

· Promoting participant safety, independence, rights and wellbeing at all times.

· Flexibility and willingness to support participants across a range of environments and activities.

Essential Requirements

Applicants must hold

· Current First Aid and CPR certification.

· Current NDIS Worker Screening Check.

· Working with Children Check, where applicable.

· Current Australian driver’s licence.

· NDIS Worker Orientation Module Certificate.

· Reliable and appropriately insured vehicle suitable for participant support.

· Certificate III in Community Services, Individual Support, Aged Care or a relevant equivalent qualification, or currently studying towards an appropriate qualification.

Availability

Applicants must have the flexibility to work across a 24/7 roster, depending on participant needs. This may include:

· Weekdays

· Weekends

· Public holidays

· Morning, afternoon and evening shifts

· Overnight/sleepover shifts

· Rotating rosters

Shift lengths may vary according to participant support requirements and applicable employment conditions.
