Female Disability Support Worker – NDIS | Western Melbourne
Primal Care Support Services is a registered NDIS provider, providing disability services to people across Melbourne. We have a wonderful prospect for a female Mental Health/Disability Support Worker to facilitate a range of care needs for our participant, while also providing opportunities for career growth and professional development.
We are currently urgently seeking a female Support Worker to work with a participant in the WESTERN SUBURBS OF MELBOURNE.

This is an opportunity to join the Primal Care Support Services team, supporting an NDIS participant living with psychosocial and/or physical disability and mental health support needs to participate and engage in social and community activities and achieve her NDIS goals.
We are seeking an energetic, passionate and highly committed individual to be part of our growing team. We currently have a role requiring a female Support Worker to provide support to a participant in Melbourne’s Western Suburbs. Shifts will vary according to availability and the participant’s support needs.
You will also have the opportunity to build your career experience while receiving ongoing professional development and support.
Our participants have various support needs, and your role as a Mental Health/Disability Support Worker may involve assistance with accessing the community, social activities, companionship, maintaining relationships, appointments and transportation. We aim to deliver high-quality, person-centred services that meet the individual needs of our participants.

Duties and Responsibilities of a Mental Health/Disability Support Worker:
- Provide support to the participant in line with your scope of practice and NDIS guidelines.
- Empower the participant to make independent choices in her lifestyle activities to meet her daily living needs and participate socially within her community.
- Provide focused interventions, support and guidance to the participant.
- Support and assist the participant to achieve her goals in accordance with her NDIS-funded supports.
- Deliver high-quality, person-centred services to meet the participant’s needs.
- Support recreational activities and community access.
- Provide support to attend appointments.
- Assist with meal planning, grocery shopping and making healthy lifestyle choices.
- Support the participant to research and plan activities of interest.
- Assist with administration tasks and engagement with relevant government bodies where required.
- As part of this role, you may be required to transport the participant using your own vehicle.
You will also:
- Build rapport and collaborate effectively with staff, management, the participant, her family and other relevant supports to achieve positive outcomes and ensure smooth service delivery throughout the participant’s plan.
- Assist with communication with the participant’s family and support network to explore new goals and ambitions and empower the participant to work towards achieving them.
- Support the participant to develop and strengthen her skills,
independence and personal development.
- Provide regular, professional and high-quality support in line with the participant’s plan, needs, preferences and choices.
- Assist the participant to access her community, maintain social connections and participate in activities of interest.
- Implement appropriate strategies and programs to develop and enhance the participant’s skills, identify areas where additional support may be required and support opportunities to achieve her goals.
QUALIFICATIONS AND EXPERIENCE
- NDIS Worker Screening Check
- Working with Children Check
- NDIS Worker Orientation Module Certificate (if you do not have this already, you may complete it after applying)
- NDIS Worker Induction Module Certificate (if you do not have this already, you may complete it after applying)
- NDIS Enjoying Quality Safe Meals Certificate (if you do not have this already, you may complete it after applying)
- COVID-19 Infection Control Certificate
- Unrestricted Driver’s Licence
- Comprehensive car insurance
- Current vehicle registration
- First Aid & CPR Certificates
- Passport/Visa documentation where applicable
- Certificate IV in Mental Health is desirable but not required
- Up-to-date résumé, including your availability
- Ability to work autonomously
- Desired: Certificate IV or degree in Mental Health, Disability, Community Services or another relevant field
- Desired: Previous experience in a support worker role or relevant field
