The Queensland Government's Mackay Base Hospital Aged Care Assessment Team seeks applications for Senior Occupational Therapist, Senior Physiotherapist, Senior Social Worker or Clinical Nurse on a temporary basis.

You will perform complex assessments of aged clients in community and hospital settings, document data via the IAT and My Aged Care portals, and develop client-centred Support Plans within a Wellness and Reablement framework.
