We are a leading and long-standing Early Parenting Centre in Victoria with a proud history and commitment to strengthen the lives of babies, toddlers and their families. Tweddle is a registered public hospital with its head office located in Footscray offering a range of early parenting programs including residential, day long and sessional groups at our main campuses at Footscray and Wyndham.

We are looking for a Family & Community Connections Coordinator four days per week at our Footscray location to provide clinical coordination across Tweddle programs to ensure families receive seamless, tailored support from referral to discharge as well as other initiatives aligned to our strategic plan.

What you'll DO.

This position provides leadership to the Assessment & Intake and Home Visiting Teams, clinical advice for high risk and complex families across the service with the key objectives listed below:

- Improving referral pathways and strengthen wrap around supports and early interventions for families experiencing a period of vulnerability
- Provide substantial post-discharge follow up options and to progress recommendations and referrals to improve outcomes of families.




- Strengthening access to services by building partnerships with families, communities, and external providers
- Collaborate with the clinical leadership team to explore and implement innovative and agile strategies to ensure high-quality early parenting services.

What you'll bring

- A Bachelor level qualification in Allied Health or Nursing
- Strong stakeholder engagement skills and capacity to represent Tweddle in sector forums
- Experience supporting families with multiple or complex issues
