360 Health Community is a community health service with a difference. Our local teams are based in Perth, Peel, Geraldton and Kalgoorlie to bring care and support to people when, where and how they need it most through well known, trusted and successful programs such as the Street Doctor, headspace, ALIVE, Regional Mental Health Care, and Commonwealth Psychosocial Support.

The Commonwealth Psychosocial Support Program supports people who have a mental health illness which impacts on their functional capacity. The Program aims to strengthen the capacity of people with a severe mental illness and assist them to live independently and safely in their community, achieve personal recovery goals, form meaningful connections in a supportive environment, and reduce the need for acute care.

About the role

The Intake and Triage - Wellbeing Partner is primarily responsible for the intake and assessment of clients referred into the Wellbeing Pathways program. This involves conducting initial client assessments, identifying risks and support needs, and facilitating access to appropriate psychosocial and community support services. The role ensures clients receive timely, safe, and person-centred support through effective triage, risk management, and service coordination.

Requirements

- Qualifications in Mental Health, Community Services, Social Work, or other related field.
- Minimum one years' experience providing psychosocial, mental health, or community-based support in an outreach setting.
- Strong understanding of recovery-oriented, strengths-based,



and person-centred practice when supporting individuals with mental health challenges and complex needs.
- Demonstrated experience conducting intake assessments, psychosocial assessments, risk screening, and developing recovery-focused support plans.
- Knowledge of safeguarding, duty of care, and risk management.
- Excellent communication, interpersonal, negotiation, and stakeholder engagement skills.
- Proven ability to build effective relationships with clients, families, carers, service providers, and community agencies.
- Sound knowledge of mainstream health, mental health, disability, and community services.
- Strong organisational skills with the ability to manage competing priorities, referrals, and caseloads effectively.

- Valid National Police Clearance or willing to obtain
- Current Driver's Licence
- Working with Children Check

An hourly rate above award level, starting from $39.94 per hour (annual full time equivalent of $78,921.44). Extra benefits include:
