As we continue to grow our services across QLD & VIC, we are looking for a passionate and organised Support Coordinator to join our team.

This role is looking for the right candidate to provide Support Coordination and Specialist Support Coordination (Level 2 and 3) services from our Office Based in Marsden QLD.

About the Role

As a Support Coordinator, you will work closely with NDIS participants, their families, support networks and service providers to help participants understand and use their NDIS plans effectively.

You will support participants to build capacity, exercise choice and control, connect with appropriate services, and work towards their individual goals.

The successful candidate will be confident managing multiple participants, coordinating services and maintaining robust professional relationships with participants and stakeholders.

Key Responsibilities

- Coordinate NDIS supports in line with participant goals, needs and funding.
- Build strong relationships with participants, families and key stakeholders.
- Connect participants with appropriate services, providers and community resources.
- Support participants to understand and effectively use their NDIS plans.




- Monitor service delivery, funding utilisation and participant outcomes.
- Advocate for participant choice, control, rights and independence.
- Coordinate referrals, appointments, reviews and stakeholder meetings.
- Maintain accurate case notes, reports and participant documentation.
- Identify and escalate risks, safeguarding concerns and service gaps.
- Collaborate with the Divine Support Care team to ensure quality and consistent support delivery.
- Work autonomously with strong organizational skills and self motivation

Qualifications & experience

- Tertiary education in either Disability, Community Services, Social Work, Psychology or substantial industry experience
- NDIS Worker Screening Clearance
- Current Drivers License
- Working with Children’s Check
- Previous experience working as a Support Coordinator
- Strong knowledge of the disability and community services sector
- Experience working with people with disabilities, child protection involvement, homelessness, Alcohol and Other Drugs and mental health
- NDIS Online Orientation Module

Pay: $37.00 – $60.28 per hour

Expected hours: No less than 24.0 per week
