NDIS Support Coordinator / Psychosocial Recovery Coach – Full-Time
Centre of Hope is seeking an experienced, reliable and genuinely caring NDIS Support Coordinator / Psychosocial Recovery Coach to join our team on a full-time basis.
This position is suited to someone who already has experience working within the NDIS sector, is confident supporting participants with complex needs, psychosocial disability and mental health conditions, and is comfortable managing a busy caseload while meeting organisational KPIs and deadlines.
This is a work-from-home position; however, it is not a fully remote role. The successful applicant must be willing and able to travel to different areas across Sydney to visit participants, attend meetings and appointments as required.
Position Details
- Full-time position – Monday to Friday
- Working hours: 9:00 am to 5:00 pm
- Salary: $74,000 per year + superannuation
- Work from home
- Regular travel across different areas of Sydney is required

About the Role As an NDIS Support Coordinator / Psychosocial Recovery Coach, you will support NDIS participants to understand and effectively use their NDIS plans, connect with appropriate services and work towards their individual goals.
You will manage your own participant caseload and will be expected to maintain regular communication, complete required follow-ups, maintain accurate documentation and meet organisational KPIs.
As part of the Psychosocial Recovery Coaching component of the role, you will work closely with participants who experience psychosocial disability and mental health challenges. You will support participants to build capacity, improve independence, strengthen their informal and formal support networks and work towards recovery-focused goals.
You will support participants to better understand their strengths, needs and available supports, develop strategies to manage challenges and connect with suitable mental health, community and disability services.
Many of our participants have complex disabilities, psychosocial disabilities, mental health conditions, behavioural support needs or complex personal and service environments. Previous experience working with participants with mental health and psychosocial support needs is essential.
This is a busy position and applicants must be comfortable working under pressure, prioritising competing tasks and managing multiple participants at the same time.
Key Responsibilities
- Provide Support Coordination in accordance with participants’ NDIS plans, goals and funded supports
- Provide Psychosocial Recovery Coaching to participants with psychosocial disability and mental health support needs
- Manage an allocated participant caseload effectively
- Meet organisational KPIs, participant contact requirements and documentation deadlines
- Maintain regular communication and follow-up with participants and their support networks
- Support participants to understand their NDIS plans and available funding
- Support participants to build capacity and increase independence
- Work collaboratively with participants to identify recovery goals and strategies
- Support participants to develop greater confidence in managing their supports and daily life
- Assist participants to identify and strengthen formal, informal and community supports
- Support participants to engage with mental health, community, disability and mainstream services
- Connect participants with suitable disability, community and allied health services
- Support participants experiencing complex psychosocial,
behavioural and mental health needs
- Communicate professionally with participants, families, guardians, nominees and service providers
- Coordinate with occupational therapists, behaviour support practitioners, psychologists, psychiatrists, mental health teams, physiotherapists, speech pathologists and other allied health professionals
- Liaise with hospitals, community mental health teams and other services where required
- Attend participant home visits, appointments, case conferences and provider meetings when required
- Travel to different locations across Sydney according to participant needs and your weekly schedule
- Maintain accurate case notes, participant records and documentation
- Prepare professional emails, reports, referrals and NDIS-related documentation
- Monitor participant supports and identify service gaps
- Identify, document and escalate risks, safeguarding concerns or changes in participant circumstances
- Support participants during periods of change, transition or increased support needs
- Manage competing priorities and urgent participant matters professionally
- Work closely with and report to the Support Coordination Team Leader
- Follow Centre of Hope policies, NDIS requirements, privacy requirements and qualified boundaries
- Provide services using a participant-centred, recovery-oriented and strengths-based approach
Mandatory Requirements Applicants must meet the following requirements:
- Previous experience working within the NDIS sector
- Experience working with people with disability, preferably within Support Coordination, Psychosocial Recovery Coaching, mental health services or a similar NDIS role
- Experience supporting people with mental health conditions and psychosocial disability
- Minimum Certificate III in Disability, Community Services, Mental Health or another relevant community services qualification
- Ability to independently manage a participant caseload
- Ability to work with participants who have complex psychosocial and mental health support needs
- Understanding of recovery-oriented and person-centred practice
- Ability to work under pressure and manage competing priorities
- Ability and willingness to work towards and meet organisational KPIs
- Strong verbal and written communication skills
- Good computer, administration and documentation skills
- Current Australian Driver Licence
- Reliable own vehicle
- Willingness and ability to travel to different areas across Sydney during working hours
- Current NDIS Worker Screening Check or willingness to obtain one
- Working with Children Check where required
- Australian working rights

Who This Role Will Suit This is a busy and responsible role and is best suited to someone who already understands the NDIS workplace and has experience supporting people with disability, mental health conditions or psychosocial disability.
The successful applicant must be resilient, dependable and comfortable managing a caseload, working under pressure, meeting KPIs and responding to competing participant needs.
Experience within mental health, psychosocial disability, Support Coordination or Psychosocial Recovery Coaching will be highly regarded.
Although you will primarily work from home, regular participant contact is an important part of the role. You must be comfortable travelling to different areas across Sydney for participant visits, meetings and appointments.
Full training, handover and ongoing Team Leader support will be provided; however, applicants are expected to already have relevant NDIS sector experience and a good understanding of professional practice within disability, mental health or community services.
