Independent Living Support Association (ILSA) is a not-for-profit community organisation providing quality support services to people with disability and older Australians living independently.

n

ILSA Home Care Services supports older Australians receiving Support at Home funding to remain living safely, independently and well in their own homes and communities.

n

We are seeking an experienced and values-driven Clinical Nurse to join our Aged Care team for three days per week.

n

This is a varied community-based role combining hands‐on nursing care, clinical assessment and care planning, clinical oversight, staff education and competency assessment, and contribution to clinical governance and quality improvement.

n

It would suit an experienced nurse who enjoys working independently, building relationships with clients and families, and using their clinical expertise to influence the quality of care across a broader service.

n

About the role

n

As ILSA's Clinical Nurse, you will work closely with clients, families, Care Partners, support workers and other health professionals to ensure clinical care is safe, responsive and genuinely person‐centred.

n

Your role will include:

n

Undertaking comprehensive clinical assessments and reassessments for older people living in the community;

n

Developing, reviewing and updating clinical care plans and risk assessments;

n

Providing direct nursing care in clients' homes, including wound assessment, wound care and ongoing wound management;

n

Identifying and responding to changes in clients' health, functioning and clinical needs;

n





Providing clinical support relating to medication management, continence, skin integrity, falls, chronic health conditions and other nursing needs;

n

Coordinating appropriate clinical escalation, referral and follow‐up;

n

Liaising with GPs, hospitals, pharmacists, allied health professionals, families and other service providers;

n

Providing clinical guidance and support to Care Partners and support workers;

n

Supporting the safe delivery of clinical and higher risk supports;

n

Providing education, training and competency assessment to support workers;

n

Reviewing clinical documentation, incidents and emerging risks;

n

Contributing to clinical governance, quality improvement, audits and the development and review of clinical policies and procedures;

n

Promoting dignity of risk, informed choice and independence while ensuring clinical risks are appropriately identified and managed.

The role will primarily support clients in Cairns, with occasional travel to clients on the Atherton Tablelands where required.

n

About you

n

We are looking for someone who brings strong clinical judgement together with a practical, respectful and person‐centred approach.

n

You will:

n

Hold current registration with AHPRA as a Registered Nurse;

n

Have significant nursing experience, ideally within aged care, community nursing, home care or a related setting;

n

Be confident working autonomously and providing nursing care in clients' homes;

n

Have strong clinical assessment and care planning skills;

n

Have experience in wound assessment and wound management;

n





Be confident identifying and responding to clinical deterioration and changing health needs;

n

Have sound knowledge of the clinical issues commonly experienced by older people living in the community;

n

Be comfortable providing clinical guidance, education and competency assessment to support workers;

n

Have an understanding of clinical governance, risk management and quality improvement;

n

Be able to balance dignity of risk and client choice with sound clinical judgement and duty of care;

n

Be an excellent communicator who can build respectful relationships with clients, families, workers and health professionals;

n

Be organised, approachable, responsive and comfortable working both independently and as part of a collaborative team.

A current driver's licence and ability to travel locally are required.

We offer

$58–$62 per hour, depending on qualifications and experience;

Classification at Registered Nurse Level 3 under the Nurses Award 2020, with remuneration above the Award;
