Austin Health, Melbourne, is seeking a Nurse Practitioner to lead clinical care within the Community Transition Care Program. Based at Heidelberg Repatriation Hospital, you will work with clients in the community and provide advanced nursing assessment and management to improve transitions of care.

You will lead rapid clinical interventions, support governance, and coordinate care across the TCP team to reduce hospital presentations and improve discharge planning, while aligning with Aged Care.
