Locum Clinical Pharmacist - Grampians Health, Ballarat

About the Role

n

Grampians Health is seeking experienced Clinical Pharmacists to join the pharmacy team at Ballarat Health Services on a locum basis. Working within a supportive, multidisciplinary environment, you will provide high-quality ward-based clinical pharmacy services across a range of inpatient specialties.

n

The successful candidate will become part of a well-established team of pharmacists delivering patient-centred care across one of regional Victoria's largest health services. Opportunities may exist to work across a variety of clinical areas depending on experience and service requirements.

Key Responsibilities

n

n
- Deliver comprehensive clinical pharmacy services to inpatient wards, including medication reconciliation, medication review, therapeutic recommendations and discharge planning.
n
- Provide clinical assessment, intervention, advocacy and consultation to patients and the multidisciplinary team.
n
- Support safe and effective medication management practices in accordance with hospital policies and professional standards.
n
- Facilitate continuity of care and contribute to effective patient flow, particularly at transitions of care.
n
- Liaise with medical, nursing and allied health staff to optimise medication outcomes for patients.
n
- Participate in ward rounds and multidisciplinary meetings as required.
n
- Provide education and mentorship to pharmacy students, interns and junior staff.
n
- Contribute to departmental quality improvement initiatives, education programs and clinical governance activities.
n




- Support operational pharmacy services and weekend/public holiday rosters as required.
n

Essential Criteria

n

n
- Current general registration as a Pharmacist with AHPRA.
n
- Previous experience working in an Australian hospital.
n
- Demonstrated ability to provide ward-based clinical pharmacy services independently.
n
- Excellent communication and interpersonal skills, with the ability to work effectively within multidisciplinary teams.
n
- Strong organisational and time-management skills.
n
- Ability to adapt quickly to new clinical environments and hospital systems.
n

Desirable Criteria

n

n
- Experience across acute medical, surgical or specialty wards.
n
- Previous locum or regional hospital experience.
n
- Experience with electronic medication management systems.
n
- Partnered Pharmacist Medication Charting (PPMC) credentialing or willingness to undertake training.
n
- Postgraduate qualifications or ongoing professional development in clinical pharmacy.
n
