Through Western Health’s unique Westwards program, you can combine your passion for rehabilitation, GEM, and transitional care with the opportunity to travel across regional Victoria—all anchored by a permanent contract with Western Health.

As a Sub-Acute Westwards Registered Nurse, you will work within Western Health’s sub-acute clinical units before undertaking supported secondments to partner sub-acute services across regional, rural, and coastal Victoria. You will bring vital restorative nursing expertise to communities where resources are needed most, while receiving complete accommodation support, meal allowances, and clinical backing.

Your Scope & Clinical Impact

Working collaboratively across diverse multidisciplinary teams, your responsibilities include:

- Restorative Care Delivery: Lead goal-directed nursing interventions across rehabilitation, geriatric evaluation and management (GEM), and complex transitional care beds.
- Regional Secondment Assignments: Undertake planned deployments to regional and rural sub-acute facilities across Victoria, adapting seamlessly to local clinical models.
- Holistic Multidisciplinary Planning: Collaborate closely with allied health professionals, geriatricians, families,



and community providers to ensure safe, sustainable discharges.
- Clinical Governance & Mentorship: Apply evidence-based nursing interventions, uphold strict clinical risk protocols, and support junior nursing colleagues and students.

What You Bring

- AHPRA Registration: Current, unrestricted registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA).
- Sub-Acute Experience: Minimum 12 months' recent clinical experience within an Australian sub-acute or rehabilitation setting (e.g., Rehab, GEM, or Palliative).
- Compliance Checks: Current NDIS Worker Screening Clearance (mandatory for sub-acute streams), National Police Check, Working with Children Check, and Immunisation Clearance prior to start.
- Adaptable Mindset: Confidence and composure when stepping into diverse rural and regional hospital wards.
- Roster Availability: Ability to work a 7-day rotating roster and participate in ongoing regional secondments.
