Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.

Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Description

The pilot is underway and the multidisciplinary team is in place. We’re looking for an experienced Project Lead to drive delivery, strengthen integrated ways of working, and turn learning into meaningful outcomes and a clear pathway for the future.

The Opportunity:

- Four days per week (0.8 FTE)
- Fixed-term contract to July 2027, with potential for extension
- Competitive salary plus not-for-profit salary packaging
- Hybrid working across our Box Hill and Eltham sites
- Lead an innovative pilot aligned with leading practice and contemporary models of care

Skills And Experiences

What You’ll Bring:

- Experience leading complex health programs, pilots, or new service models
- Strong project delivery and continuous improvement skills
- Experience leading multidisciplinary teams
- The ability to build trusted partnerships across health and community services
- Confidence managing outcomes, data, budgets, risk and executive reporting
- A cooperative, adaptable leadership style that supports clinical teams to test, learn and adapt new models of care
- A relevant qualification in allied health, nursing, health management, public health or a related field

Experience in chronic conditions, integrated care, hybrid delivery, healthcare funding, or evaluation partnerships will be highly regarded.
