Job Description

NSW Justice Health & Forensic Mental Health Network is seeking an Aboriginal Health Worker/Practitioner, Primary Care - PFT - Junee to deliver patient-centred care within the correctional health context.

n

The role focuses on culturally secure, holistic care, and prevention/management of chronic disease for Aboriginal clients, in collaboration with a multidisciplinary team to support transitions from custody back into the community.
