The role balances 0.5FTE Social Work Lead and 0.5FTE Senior Social Worker, delivering social work assessment, discharge planning and collaborative care with multidisciplinary teams.

You will influence practice, support workforce development and drive service improvement within the Northern Adelaide Local Health
