About the role

As a Level 2 Support Coordinator, you will provide high-quality, participant-centred Support Coordination that enhances choice, control and independence.

Key responsibilities

- Implementing participants' NDIS Plans and supporting them to achieve their identified goals
- Supporting participants to understand their NDIS funding and maximise the value of their available supports
- Building participant capacity to navigate the NDIS and coordinate their own supports wherever possible
- Managing an appropriate caseload in accordance with your agreed working hours
- Connecting participants with suitable formal, informal, community and mainstream supports
- Maintaining strong relationships with participants, families, nominees, service providers and other stakeholders
- Regular communication with allied health professionals, NDIA representatives and LACs
- Conducting home visits and face-to-face participant and stakeholder meetings
- Preparing implementation, progress and plan reassessment reports within required timeframes
- Maintaining accurate and timely case notes and records of Support Coordination delivered

About you

- Experience in NDIS Level 2 Support Coordination
- Strong and current knowledge of the NDIS
- Excellent stakeholder engagement and relationship-building skills
- Robust written communication and NDIS report-writing skills
- Excellent case management,



organisation and time-management skills
- Ability to work independently while being part of a supportive team
- A participant-centred approach with a strong commitment to choice and control
- Understanding of safeguarding, privacy, professional boundaries and conflicts of interest
- Confidence communicating with participants, families, NDIA, LACs, allied health professionals and service providers
- Relevant qualifications in Community Services, Disability, Social Work, Human Services or a related discipline will be highly regarded
