About the role The Support Worker/Mentor will provide individualised support and mentoring to participants across community, home and other appropriate settings. The role will involve supporting participants to achieve their personal goals, increase independence, participate in their community and develop positive connections. You will be someone who can build trust, maintain professional boundaries and create positive relationships through consistency, respect and understanding.

Key responsibilities

- Provide culturally safe, respectful and person-centred support and mentoring.
- Build positive, professional and trusting relationships with participants.
- Support participants to work towards their individual goals and aspirations.
- Encourage independence, self-determination, confidence and decision-making.
- Support community access, social participation and connection to community.
- Assist with appropriate daily living activities and practical tasks.
- Provide transport and support participants to attend appointments, activities, education, employment and community events where required.
- Support participants to develop life skills, social skills and positive routines.
- Maintain professional boundaries and uphold participant dignity, privacy and confidentiality.
- Complete clear, accurate and timely support notes, incident reports and other required documentation.

- Working with Vulnerable People Check
- Police Clearance
- Current First Aid & CPR certification
- Responding to Risks of Harm, Abuse and Neglect – Education and Care (RRHAN-EC)
- Current driver's licence
- Access to a reliable, registered and insured vehicle
- Has a strong interest in supporting people to achieve their goals.
- Is confident building relationships with children, young people and adults.
- Has strong communication and interpersonal skills.
