About the role

n

Permanent Part time - 2 Nurse Practitioner Primary Health Care positions available - hours negotiable

n

Function autonomously and collaboratively in an advanced and extended clinical role to provide a holistic Nurse Practitioner led health service with a specific emphasis on people who use substances.

n

You will play a crucial role in providing high-quality, person-centred care to a diverse range of people. This will be provided via face-to-face service delivery from QuIHN's Brisbane based service. Service delivery will also be provided via face to face and telehealth services to QuIHN's locations throughout Queensland as well as outreach at more remote locations.

- Provide a client focused, multidisciplinary nurse practitioner range of services in line with current best practice and person-centred care: -
n
- Work with local hospitals to provide follow-up and post discharge care
n




- Provide referral and linkage to General Practices (GP's, primary mental health, drug and alcohol, allied health and specialist services)
n
- Liaising with and advocacy to access hospital and primary care services
n
- Direct nursing interventions which may include:
n
- Comprehensive health assessments and provide person-centred primary health care, including management of chronic conditions and Blood Borne Virus (BBV) and Sexually Transmitted Infection (STI) treatment where appropriate
n
- Prescribing medications and management
n
- Wound care and vein care for those who inject, including the management of injection related injuries and infections
n
- Health education and promotion
n
- Referral and specialist triage
n

What we're looking for

n

A passionate individual dedicated to health outcomes for clients and communities, who can bring:

n

n
- Registered Nurse holding a current practice certificate as a Division 1 from the relevant authority (Nursing and Midwifery Board) endorsement as Nurse Practitioner
n
- Postgraduate qualification(s) in Nursing, Master's Degree and other credentials relevant to scope of practice
n
- Demonstrated experience providing advanced clinical nursing care, including comprehensive assessment, interpretation of diagnostic findings, treatment planning, and referral across diverse and complex clinical presentations. A strong primary health care focus is essential,



with an interest in BBV and STI care or a willingness to expand scope of practice in these areas.
n
- Demonstrated understanding of the health care delivery system, ability to lead and implement changes, and develop direction in clinical practice by the initiation, utilisation and review of evidence-based research
n
- Ability to work with a high degree of autonomy and accountability while also working collaboratively in a multidisciplinary and multi service environment.
n
- Is an active ally to the community that uses substances, and can engage effectively with vulnerable, complex individuals and families impacted by substance use and related challenges.
n
- Current professional indemnity insurance
n
- Current Queensland driver's licence (or equivalent)
n
- Willing to travel throughout Queensland, both by car and air, to provide services
n

Salary and Benefits

Remuneration based on skills and experience

- NP 1 = $79.52
n
- NP 2 = $83.00
n
- Permanent Part time role - Hours Negotiable - No after hours, weekend, or public holiday work
n
