About the role This rewarding role offers the chance to work closely with a multidisciplinary team to deliver comprehensive, patient-centred care that supports improved health outcomes, quality of life and informed decision-making for older people and their families. You will provide ongoing management of acute and chronic health conditions, preventative healthcare, medication management and clinical oversight throughout a client's stay.

About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Treat patients and their families with courtesy and sympathy and treat all hospital staff and professional colleagues with courtesy and respect

Be contactable by phone during business hours for advice and guidance on patient management.

Undertake a global health assessment at the time of the client's entry to the service





Continue close collaboration with specialists involved in clients' care

Provide ethical decision making in the achievement of organisational goals
