As a Clinical Nurse – Community Health, you'll play a vital role in improving health outcomes for people across the community. From providing advanced clinical care and health education to working alongside multidisciplinary teams, you'll help individuals and families navigate their health journey while contributing to a service that is focused on prevention, wellbeing and quality care.

Every day, you'll contribute by:

- Delivering holistic assessments, care planning and coordinated support that empowers people to achieve better health outcomes.
- Building trusted relationships with individuals, families, community partners and healthcare professionals to provide responsive, person-centred care.
- Promoting health, wellbeing and self-management through education, screening and early intervention initiatives.
- Supporting service excellence through clinical leadership, quality improvement activities and evidence-informed practice.

You're a nurse who combines clinical expertise with genuine compassion for the people and communities you serve. You bring integrity to decision-making, dignity to every interaction and the courage to lead when it matters most. Whether you're supporting a family through a complex health challenge or working with colleagues to improve care delivery, you find purpose in creating positive and lasting outcomes for others.

Here's what will help you thrive in this role:

- Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia and a current practising certificate.
- Strong clinical assessment, care coordination and problem-solving skills in a community or rural healthcare setting.
- The ability to build collaborative relationships and work effectively within multidisciplinary teams.
- A commitment to quality, safety, continuous improvement and professional growth.

You'll stand out from the crowd if you have qualifications in rural and isolated practice, immunisation, and/or postgraduate study relevant to community health.
