What you'll be doing?

- Assessing and coordinating client care — Assessing, coordinating, and managing care services for Support at Home clients to ensure their individual health outcomes are met.
- Delivering holistic care — Providing comprehensive, safe, and evidence-based clinical and nursing care with a robust focus on each client's specific needs.
- Coordinating assessments and care planning — Leading the development and coordination of care assessments and care plans tailored to individual client circumstances.
- Collaborating with internal teams — Working closely with the service coordination, finance, and intake teams to ensure smooth rostering, reimbursements, and onboarding of new clients, allowing you to focus on quality case and client management.
- Upholding quality standards — Ensuring all care delivery aligns with the Aged Care Quality Standards and best practice principles.




- Building strong client relationships — Engaging with clients and their families with empathy and professionalism, ensuring a positive and supportive care experience throughout.

Who are we looking for?

- Relevant care qualifications — A qualification in Care with community experience, such as a Certificate IV in Aged Care, Home and Community Services, Care Management, or Nursing, is required.
- Community or case management experience — Demonstrated experience in a community, case manager, or nursing role, with a strong understanding of coordinating assessments and care planning.
- Aged care quality knowledge — Experience in care management and a solid understanding of the Aged Care Quality Standards.
- Problem-solving and leadership skills — Strong problem-solving ability combined with natural leadership qualities to guide and support positive client outcomes.
- Organisational skills — A high level of organisation and attention to detail, with the ability to manage multiple clients and priorities effectively.
- Communication skills — Excellent verbal and written communication skills to engage confidently with clients, families, and internal teams.
- Bilingual language skills — Bilingual ability is essential for this role, with Mandarin as a compulsory language, supporting effective communication with a culturally and linguistically diverse client base.
- Driver's licence and vehicle — A valid Australian driver's licence and access to your own vehicle are required,



as the role involves travel to client locations across Sydney.
- Computer literacy — Competent computer skills, including Microsoft Office, along with the ability to quickly learn new business systems.
- Support at Home program familiarity — Familiarity with the Support at Home program is highly regarded, with previous experience in a home care environment considered a bonus.
