What you will do
n
As assessment and transition coordinator, you will help people prepare for life after prison.
n
You will assess risks, support parole applications and coordinate transition services.
n
You Will

coordinate risk and needs assessments, parole applications and pre-release services.
n
support case management reviews and help track each person's progress.
n
coordinate programs,



referrals and services that support people before and after release.
n
work with prisoners, prison staff and service providers to support transition plans.
n
maintain clear records, data and schedules to support programs and parole applications.
n

About The Candidate
n
You should have experience in, or an understanding of, working in a correctional environment.
n
You should understand the needs of people moving through prison and back into the community.
n
You Should Also Have
n
n
understanding of transition and reintegration issues affecting people in custody.
n
skills to use risk assessment tools with individual prisoners.
n
ability to work with people who may be hard to work with.
n
computer skills using programs such as Excel to manage data.
