Calvary Home Care in Darwin is seeking a dedicated Case Manager to oversee a caseload under CHSP/HCP funding, maintain budgets, onboard current clients, and coordinate care to support client independence.
The role requires leadership experience in case management and a strong understanding of aged care, with liaising across stakeholders and internal networks.
A driver's license and willingness to obtain ND/Police clearances are beneficial.
