Care Partner
to join our team.
This role is suited to someone with previous experience in
aged care, home care, case management or care coordination
, who enjoys building strong relationships with clients and families and coordinating quality services.
Mandarin and/or Cantonese language skills are highly regarded.

About the Role
As a Care Partner, you will support a portfolio of clients and work closely with clients, families and service providers to ensure their care needs are understood and appropriately coordinated.
Your key responsibilities will include:
Building strong relationships with clients and their families
Coordinating and reviewing services based on individual needs and goals




Supporting care planning and ongoing service arrangements
Monitoring client funding and service utilisation
Liaising with support workers, allied health professionals and other service providers
Completing regular client reviews and maintaining accurate documentation
Responding to changes in client needs and arranging appropriate follow-up
About You
We are looking for someone who has:
Previous experience in
aged care, home care, care coordination or case management
Strong communication and relationship-building skills
Good organisational and time-management skills
Confidence managing a client portfolio independently
Positive computer and documentation skills
A professional, respectful and person-centred approach
Ability to work effectively with clients, families and multidisciplinary teams
Experience with
Support at Home or Home Care Packages
will be highly regarded.
Language Skills
Due to our client base, candidates who can communicate in
Mandarin and/or Cantonese
will be highly regarded.
Strong English communication skills are also required.
