Job Description
Uniting in Horsham, Victoria seeks a part time Step Thru Care intake clinician to support people with mental health and AOD needs.
You will be the first point of contact, performing intake and assessment with a trauma-informed, recovery-focused approach across a stepped care model.
You will deliver care face-to-face, outreach and telehealth, working in partnership with Grampians Community Health to ensure access to AOD services, low- and high-intensity supports as required.
