About the role
We are seeking a dedicated Support at Home Care Partners to join our expanding team in a full-time capacity.
This role is ideal for an individual with a passion for aged care and prior experience in coordinating Support at Home/Home Care Packages.
As a Care Partner, you will play a pivotal role in delivering person-centered case management services to our clients.
You will work closely with the Manager, Nurses, and Support Workers, providing guidance, support, and training to ensure high-quality care outcomes.
This position operates under a versatile, hybrid work model, with a blend of working from home and from our Chermside office.
You will also spend part of your time visiting clients in the North Brisbane area to support their needs and maintain a strong connection with the community.
Specific tasks
Act as the key contact person and advocate for clients in facilitating their Care Plan
In partnership with the client, develop a goal-focused and consumer-directed Care Plan
Ensure client needs and preferences are provided within their allocated budget.
Organise client services and report any issues that arise to the Home Care Package Manager.
Liaise with referring agencies, ACAT,



local health providers, and community services.
Provide day to day support advice and assistance to Home Care Package Manager, nurses and Support Worker's.
Perform any other reasonable duties as directed by Care Coordination Manager.
Identify additional resourcing needs and provide recommendations to the Care Coordination Manager.
Skills & Experience
Diploma in Aged Care, Community Care and/or Case Management desirable
Minimum Certificate IV in Aged Care, Community Care and/or Case Management with experience preferred years
Working knowledge of the Strengthened Aged Care Quality Standards
Ability to develop, track and understand budgets and financial information
Previous experience in a care coordination role preferred
Ability to develop, track and understand budgets and financial information.
Experience in conducting risk assessments.
Current National Police Check
Current CPR & First Aid
