Care Partner / Clinical Partner – Support at Home (Full Time)

The Butler Canberra is looking for an experienced and passionate Care / Clinical Partner (ideally with qualifications in Allied health / Nurse / Physiotherapist etc but not necessary) to join our team and lead the delivery of high-quality, person-centred home care services. This is a pivotal role for someone who thrives in the community setting and is ready to support both our participants and our organisation through the changing aged care landscape.

Your Role
n
As one of our Care / Clinical Partners, you will:
n
n
Support the delivery of high-quality, in home clinical care through the Support at Home Program, enhancing participant wellbeing and independence.
n
Oversee the care operations for participants receiving Support at Home and private services.
n
Ensure participant assessments, care plans, and service delivery are not only clinically sound but also aligned with regulatory requirements under the new Aged Care reforms.
n
Act as a care leader and key contact for both internal staff and external stakeholders including GPs, allied health, and families.
n
Support governance and continuous improvement by assisting with internal audits, incident management, care documentation reviews, and quality initiatives.
n
Mentor support workers and case partners in clinical understanding, escalation protocols, and best-practice service delivery in the home.

Key Responsibilities
n
n
Conduct and review in-home assessments and develop tailored care plans and budgets in consultation with participants and families.
n
Facilitate all necessary care to participants.
n
Ensure compliance with the Aged Care Quality Standards and support alignment with the new Aged Care Act reforms.
n
Maintain accurate, timely clinical documentation and oversee risk and incident reporting.
n




Liaise with funding bodies and regulatory agencies as needed.
n
Work collaboratively with the leadership team to improve systems, governance, and service delivery models.
n
Provide education, mentoring and support to frontline staff and current starters.
n
Monitor and manage clinical risks.
n

What You Bring
n
n
Diploma level qualification in Community Services, Aged Care, Case Management or equivalent experience.
n
Preferred allied health background preferred (Registered Nurse (AHPRA) / Physiotherapist / Allied Health Professional) but not necessary with a minimum of 3 years' experience, ideally in aged care or community health.
n
Previous experience in case management, clinical coordinator, or team leader role within home care is highly regarded.
n
Strong understanding of the Support at Home Program and associated compliance requirements.
n
Minimum 2+ year management experience with Home Care Packages / Support at Home is preferred.
n
Excellent communication, leadership, and time management skills.
n
Compassionate, adaptable, and confident, working autonomously in the field.
n
Current driver's licence and reliable vehicle.
n
Australian work rights are required for this position.
n
Solid understanding of the Support at Home Program, Quality Standards and Aged Care Rights.
n
Skilled in using case management or CRM systems.
n
