The Role
n
We're seeking an experienced Team Leader Care Management to lead our team of 11 Care Partners across 600 aged and disability home care clients.
n
This is a genuine people-leadership role. You'll provide coaching, direction and support to your team while ensuring care management practice is consistent, compliant and outcomes focused.
n

Key Responsibilities
n
n
Lead, coach and support the Care Management team, fostering a positive, collaborative and high-performing culture.
n
Provide regular supervision and professional development to build staff capability.
n
Manage team capacity, workflow and priorities to ensure timely, consistent care management.
n
Monitor performance and care management KPIs,
identifying opportunities to improve quality, efficiency and outcomes.
n
Contribute to reporting on outcomes, risks and improvement opportunities.
n
Support the implementation of care management policies, procedures and frameworks.
n
Contribute to quality assurance activities, including audits, reviews and improvement initiatives.
n
Ensure practices align with Support at Home, NDIS requirements and organisational policies.
n
Guide the team in managing complex client needs, risk identification, escalation and advocacy.
n
Oversee the documentation, investigation and resolution of client incidents and complaints.
n
Work collaboratively with intake, scheduling, clinical and finance teams to support a coordinated client journey.
n
Support the team to build strong relationships with clients, carers, families and external providers.
n

About You
n
You'll bring:
n
n
Significant experience in care management, case management or service coordination within aged care and/or disability services.
n
Demonstrated leadership experience, with a robust track record of developing staff capability and team performance.
n
Sound knowledge of the Support at Home program, NDIS and person-centred care principles.
n
Strong understanding of the Aged Care Quality Standards and NDIS Practice Standards.
n
Experience managing and investigating incidents and complaints within a regulated service environment.
n
Strong organisational, problem-solving and time-management skills, with the ability to manage competing priorities.
n
Excellent communication and stakeholder engagement skills, with the ability to work effectively with clients, families, staff and external partners.
n
Confidence using client management systems and reporting tools.
n
A high level of attention to detail and a commitment to accurate documentation.
n
A current NDIS Worker Screening Check and Working with Children Check, or willingness to obtain.
n
Empathy, cultural sensitivity and an understanding of the unique needs of Holocaust survivors and the Jewish community.
n
A Diploma in Community Services, Aged Care, Case Management or a related discipline, or equivalent experience.
n
A clinical background.
