About the opportunity
We are seeking adedicatedServiceCoordinator(CarePartner)to playa central rolein coordinatinghighquality,clientdirectedcarefor Bunji participants.
This position ensures that every participant receives services aligned with their individual goals, preferences, and funding requirements.
This is apermanent,onsiterolethat may also involve participant visits and occasional direct service delivery to support personalised care.
TheService Coordinatorwill:
Oversee tailored, participantcentred care across individual and group settings
Manageparticipantonboarding,goalsettingand care planning activities
Work with broader support team to align service delivery with participant goals and budget requirements
Coordinate supports for participants with complex care requirements, including those funded through NDIS, iCare,NationalInjuryInsuranceSchemeandother rehabilitation or compensable schemes
Support implementation of complex support plans, behaviour support plans, manual handlingplansand clinical requirements
Developandmaintainpathwaysand relationships with key organisationsandindustrybodiestoinfluence schemeand business outcomes
Assess and managerisks to participantsand the organisation
Support effective management of client budgets and funding arrangements
Manage participant experience and complaintresolutionprocesses.
Who we are looking for
We are seeking someone who brings:
acommitment to supporting the rights, dignity, and independence of people with disabilities;
excellent communication andrelationshipbuildingskills;
strong organisation skills and confidence inmanaging competing priorities in a fast pacedenvironment;




demonstrated digital competency and experience in use of digital systems and NDIS frameworks
strongassessment, planning, andproblem-solvingabilities;
leadership competencies and ability to support, influenceand coordinatethetasks of others;and
empathy, patience, adaptability, and a collaborative mindset.
The successful candidate will also have:
Certificate IVor higher in Disability Individual Support, Community Services or a relevant Social Services qualification
Extensive experience inDisability,CommunityServices, or Case Management
Demonstrated experience in asimilarParticipant/ServiceCoordinationrole
Understanding of NDIS legislation, funding structures, and participant pathways
Experience supporting participants with complex conditions such asacquiredbrain injury (ABI), complex physical disability,psychosocialdisabilityorbehaviours of concern.
Solid digital competency, including client record systems and scheduling tools
All Bunjiemployeesarealsorequiredtomeet the following compliance:
Hold acurrent NDIS Worker ScreeningCheckandWorkingWithChildrenCheck
Hold avalid Driver's Licence and access to a reliable, insured vehicle
Have had a minimumtwo doses of COVID19 vaccination and willingness to receive an annual flu vaccination
Holda valid Right to Work in Australia
