About the Role
As theClinical Care Coordinator, you will play a key role in supporting the delivery ofsafe, effective and person-centred clinical careacross the home.
This full-time position combinesclinical leadership, hands-on nursing care, care planning, mentoring and staff support, while working closely with residents, families, healthcare professionals and the leadership team.
You will have the opportunity tolead, mentor and support the care team, contribute to positive resident outcomes and help maintain high standards of clinical practice within the home.
The role is particularly suited to an experienced Registered Nurse who is ready tostep up, take on greater responsibility and have their clinical expertise recognised.
Key Responsibilities
Coordinatethe admission of new residents, including comprehensive clinical assessments and development of person-centred care plans
Lead, mentor and supportthe care team to deliver protected, high-quality clinical care
Provide hands-on nursing care, including medication administration, wound management and ongoing clinical monitoring




Maintainaccurate and comprehensive clinical documentation
Overseeinfection prevention and control practices
Ensurecompliance with relevant regulatory standards and accreditation requirements
Communicateeffectively with residents, families and external healthcare providers
Collaboratewith multidisciplinary care teams to achieve positive resident outcomes
Contributeto quality improvement initiatives and ongoing clinical excellence
Supporta culture of teamwork, genuine connection and person-centred care

About You
You are askilled, dedicated and clinically capable Registered Nursewho is ready to take the next step in your aged care career.
You will also bring:
Bachelor of Nursingwith current AHPRA registration
Minimum2 years' nursing experience in residential aged care
Experience in a similar clinical care role will be highly regarded
Strong clinical assessment, documentation and evidence-based practice skills
Demonstrated ability to work collaboratively within a multidisciplinary care team
Excellent communication and interpersonal skills
Confidence engaging with residents, families, colleagues and external healthcare professionals
Proficient computer and information management skills, including Microsoft applications and clinical software systems
Sound understanding of theaged care accreditation process
Ability to meet relevantAged Care Worker Screening requirements
A genuine commitment to delivering compassionate, safe and person-centred care
