About the Role
n
The Clinical Care Coordinator is responsible for overseeing the delivery of high‐quality clinical care within the community setting.
n
You will be the go‐to‐support for the nurses and carers in the field, ensuring they have the support, guidance, and clinical expertise needed to deliver excellent care, a broad range of clinical skills is required for this position.
n
Community nursing experience is a requirement for this position.
n
Key Responsibilities
n
n
Provide clinical oversight and guidance to nurses and carers working in the community.
n
Assess client needs and collaborate with families and healthcare professionals.
n
Develop clients Care plans liaising with relevant services and individuals
n




Managing acute nursing care from clients transitioning from hospital care to in‐home care
n
Maintain accurate client records and ensure compliance with care standards.
n
Lead quality improvements and service enhancements.
n

What We're Looking For
n
n
Current AHPRA registration (Registered Nurse)
n
Minimum 3 years' experience in a community‐based clinical Or a clinical coordination role
n
Strong clinical skills and experience in case management
n
Ability to prioritise and respond to complex care needs
n
Strong communication, negotiation, and conflict resolution skills
n
n
Required Certifications
n
n
Current CPR and Manual Handling Certification
n
NDIS Screening Check, Working with Children Check, and Police Check
n
n
