What You’ll Be Doing Depending on the role, responsibilities may include:
Managing Support at Home / home care services
Leading and supporting care teams
Managing client relationships and family engagement
Coordinating in-home care and community services
Developing and reviewing care plans and support plans
Managing client intake, assessments and service coordination
Monitoring quality, compliance and client outcomes
Managing incidents,



risks, complaints and feedback
Working alongside RNs, ENs, PCAs/PCWs, support workers and allied health professionals
Supporting workforce planning, rostering and staff performance
Building relationships with clients, families, stakeholders and external providers
Ensuring services align with Aged Care Quality Standards, legislation and organisational requirements
Driving continuous improvement and quality care

What We’re Looking For

We’d love to speak with people who have experience in:

Home Care | Support at Home | Aged Care | Community Care | Community Services | Care Management | Case Management | Care Coordination | Client Services | Operations | Team Leadership

Experience with HCP, CHSP, Support at Home, NDIS or community-based care is highly regarded.
