The Adult Community Health Service provides multidisciplinary, person-centred care across a range of community-based programs and clinical services. These include Care Coordination, Community Rehabilitation, Post-Acute Care, Rapid Response Falls, the Queensland Ambulance Service Falls Co-Responder service, Pulmonary Rehabilitation, Persistent Pain Management, Refugee Health, Aboriginal and Torres Strait Islander Health Worker services.
The multidisciplinary Rapid Response Falls Service provides a community based integrated pathway for older adults at risk of falls who are living in the Cairns region to access rapid in-home assessment, tailored interventions, and linkages to existing services to reduce risk of falls and reduce conveyance to emergency departments.
About your role:
Your main tasks may include (but are not limited to):
Comprehensive in-home nursing assessments,



clinical observations and deterioration management.
Falls risk assessment and prevention.
Wound, skin integrity and clinical device care.
Care coordination, referrals and clinical handover.
Multidisciplinary assessment and care planning.
