Job Description
The Salvation Army Australia is seeking a Case Manager for the Housing Support Program in Darwin.
You will provide outreach case management, develop collaborative case plans, and help participants access community supports and services.
The role requires solid risk assessment, intake, and documentation within a trauma‐informed, inclusive framework.
You will work within the Homelessness team, reporting to the Supported Housing Senior Team Leader, and operate under SCHADS Level 4 pay.
